The purpose of this systematic review is to identify where medical errors are reported in research literature also as far as debunking the myth that the majority of medical errors are attributed to communication.

This systematic review aims to identify where medical errors are reported in research literature so as to classify and categorise medical errors and thus better channel patient safety initiatives as well as research and funding agency priorities.
